I have used deoxycholic acid, the active ingredient in Kybella, for dissolving fat almost 10 years ago. Many practitioners at the time ordered the medicine from compounding pharmacies in various concentrations. The treatments were known by various names, most commonly “LipoDissolve” or “mesotherapy” and its use for the treatment of fat reduction was “off-label.” These treatments fell out of favor after a few years because of inconsistent results, likely attributed to the off-label usage and lack of technical knowledge for attaining reliable results. For this reason, I was initially hesitant to use Kybella, but after attending their training seminars and using it first-hand, I realized a very important difference. The deoxycholic acid used years ago came in various concentrations and there was no supporting evidence beyond anecdotal reports from physicians using the prior versions of the medicine to be sure exactly which dosing concentration is ideal to administer, how much of it was to be used per injection site, or exactly how far apart injections should be spaced to produce consistent, reliable results. The large scale studies performed on Kybella to solve these problems resulted in a very promising, accurate, and potentially revolutionary solution to address a very common problem people face.
What is it?
The active ingredient in Kybella is deoxycholic acid, a naturally-occurring bile salt that aids in the breakdown and removal of dietary fat. When used as an injection into the subcutaneous fat, it acts very similar to a detergent, breaking open the fat cell contents releasing the fat, which then is naturally removed.
Why is this treatment important?
The “double chin” is influenced by several factors, such as aging, genetics, and weight gain, and is often resistant to diet and exercise alone. Up until now, there was very little we could do, non-surgically, to address this problem.
How does it work?
Kybella works by disrupting the fat cell membranes, causing them to release their fat contents, which are reabsorbed by the body’s lymphatic system and permanently eliminated. In general, our bodies contain a finite number of fat cells - that is, the fat cells we have can get bigger and smaller, but we don’t make more of them. So, the fat cells Kybella eliminates, they never return.
How many treatments does it take?
In general, most people need an average of 2 or possibly 3 treatments, spaced at least 4 weeks apart. During your consultation, your healthcare professional will give you an estimate of total number of treatments that may be required for your specific goals to help you decide if Kybella is right for you.
What happens at my treatment appointment?
The area is cleansed. The injector marks the area of treatment. A temporary, wax tattoo grid is applied to aid in a very precise injection technique. Ice is held over the treatment area immediately before the injections, which typically only take a few minutes to perform. The action of the medicine happens in the first 10 minutes after injection, although the majority of post-treatment swelling can last a few days or more. Most patients report a mild to moderate warm sensation shortly after injection that lasts approximately 5 minutes and then quickly subsides.
What can I expect after my treatment?
For most people, downtime with Kybella occurred most commonly after the first treatment session. Redness, swelling, and bruising are common side effects and typically last 3-4 days. At Anthony Loren, we take extra measures to ensure a faster resolution of any side effects by providing our patients a post-recovery kit, which includes an ice pack (to use immediately after the treatment), Arnica (a homeopathic remedy that has been shown to reduce bruising and swelling), a compression garment wrap that can help improve comfort at home, Benadryl to take the night of the treatment if there is any itching sensation, and a couple days of Zyrtec to use the subsequent days, unless contraindicated.
What happens to the skin in the neck area after the fat is eliminated?
In clinical trials, when performed on appropriate candidates for the procedure, Kybella treatments actually resulted in skin retraction (tightening)! There have been hints that the manufacturer is actually going to be seeking a second FDA indication for skin tightening, which is very promising to those who would otherwise be concerned about skin laxity post-treatment.
